When HA filler is placed too superficially under thin under-eye skin, it refracts light and appears blue. Prevention requires deep supraperiosteal placement by an experienced injector. If you develop a blue tint, hyaluronidase can dissolve the filler.

HA filler in the tear trough often lasts 1-3 years, sometimes longer. Some patients metabolize filler faster, while others see persistence beyond expected duration. We do not recommend permanent fillers in this area.

In experienced hands, on the right candidate, with conservative technique, under-eye filler is generally safe and fully reversible. The area is unforgiving, though: risks include bruising, swelling, lumpiness, the bluish Tyndall effect, migration, and, rarely but seriously, vascular occlusion, which in the periorbital area can in rare cases threaten vision. Safety here comes down to a qualified, experienced injector using small amounts, placing deep, selecting candidates carefully, and keeping emergency protocols on hand.

Pricing varies widely by region and injector experience, typically running several hundred to over a thousand dollars per syringe, and the tear trough often needs only a small amount. The most expensive under-eye filler is a cheap one placed badly: correcting puffiness, discoloration, or migration costs more in money and distress than choosing an experienced injector at the outset. Treat price as a secondary factor here, well behind skill.

If you would rather avoid injectables, several options address the under-eye depending on the cause: PRP and microneedling gradually thicken and improve thin, crepey skin; restoring mid-face (cheek) volume can soften a tear-trough shadow from below; brightening treatments fade pigmented dark circles; and lower-eyelid surgery (blepharoplasty) is the answer for protruding fat bags, which filler cannot fix and can worsen. Good skin care and sun protection support all of these.

We strongly advise against semi-permanent or permanent fillers (PMMA, calcium hydroxylapatite, polylactic acid) in the under-eye area. Complications are more difficult to treat and persist longer.
